Summit Country Day will sponsor Physical Examination Days at the Summit Athletic Complex on the following days:
Wednesday, July 22 and Thursday July 30, 2009
From 5:00 - 7:30 p.m. There is a $20 fee for the exam - Sorry, no insurance cards are accepted.
Summit Athletic Complex - 5580 Ehrling Road, Cincinnati, OH 45227
Athletes can get their annual sports physical exam on one of these two days, or can go to their own personal physician to have the athletic forms completed.
In addition, Tri Health will hold a physical exam day at the Fitness and Health Pavilion, 6200 Pfeiffer Rd, on July 18, 2009 from 8:00 - 11:00 a.m. The fee is $20.
All athletes must have an updated physical exam and turn in new forms each school year. Forms are posted on the left column of this website. Parent signatures are required.
