The 2009-2010 Athletic Forms are posted below. In order to participate in The Summit Athletics programs - please be sure to provide FIVE PAGES to the Athletic Department. The OHSAA Pre-participation Physical Form, as well as, the Emergency Medical Authorication Form are REQUIRED documents needed prior to a student athlete being able to participate in a sport.
